Ways You Can Help

The Alzheimer Society of Ontario in partnership with the Aging at Home Strategy has developed the "First Link" logic model which provides clients with coordinated support, learning and linkages to community services from the point of diagnosis by a medical doctor through to the continuum of the disease.

The coordinated approach between Family Physicians and the Society.

Please contact the office either by phone or email in order to receive more information on the "First Links" program.
